The PHP Foundation supports nonprofit organizations working to improve the health and wellness of low-income, high-risk individuals across Northeast Indiana. Funds are used to support charitable organizations in the following communities served by PHP: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wabash, Wells, and Whitley counties. Beginning in 2026, the Foundation will offer two competitive grant cycles each year—one in the spring and one in the fall—along with a rolling Emergency Grant Program for urgent needs. Collaborative applications are encouraged.
Emergency Grants
Designed to address urgent needs affecting vulnerable populations.
· Maximum request: $25,000
· Applicants must complete a brief eligibility quiz
· Minimal reporting required
Spring and Fall Grant Cycles
These competitive cycles support larger-scale programs and innovative solutions that advance health equity.
· Maximum request: $300,000
· All applicants begin with an eligibility quiz
· Eligible organizations submit a Letter of Intent (LOI)
· Selected applicants are invited to submit a full application
· Minimal reporting for most grantees (grants over $100,000 require a more detailed final report)
· Reporting includes number of individuals served, outcomes achieved, partnerships or collaborations involved
Definition of Low Income
Low income is defined as 80% of the Indiana median income per the Healthy Indiana Plan (HIP):
· $48,000 for a single person
· $64,000 for a family of four
